LuciApro 12.5 mg, containing the active ingredient aprocitentan, is a novel antihypertensive medication developed to address resistant hypertension—high blood pressure that remains uncontrolled despite the use of multiple antihypertensive agents. It is marketed under the brand name Tryvio and is available in oral tablet form.

What Is Aprocitentan?

Aprocitentan is a dual endothelin receptor antagonist (ERA) that targets endothelin-1 (ET-1), a potent vasoconstrictor involved in regulating vascular tone and blood pressure. By inhibiting ET-1 from binding to both ETₐ and ETᵦ receptors, it reduces vasoconstriction, decreases vascular resistance, and lowers blood pressure.

Mechanism of Action

One potent vasoconstrictor that helps regulate blood pressure and vascular tone is the peptide endothelin-1 (ET-1).  Aprocitentan is a dual ERA that prevents ET-1 from binding to both ETₐ and ETᵦ receptors. By inhibiting the binding of ET-1 to these receptors, aprocitentan reduces vasoconstriction, decreases vascular resistance, and lowers blood pressure.

How to Use 

Aprocitentan is prescribed to treat adult hypertension, especially in individuals with resistant hypertension, a condition in which at least three distinct classes of antihypertensive medications, usually including a diuretic, have failed to lower blood pressure.

How to Take

Recommended Dose: 12.5 mg orally once a day, with or without food.

Other Dosage

  • Renal Impairment: For mild to severe renal failure (eGFR ≥15 mL/min), no change is advised. For end-stage renal illness (eGFR <15 mL/min), it is not advised.
  • Hepatic Impairment: No dose adjustment is recommended for mild hepatic impairment (Child-Pugh class A). Not recommended for moderate to severe hepatic impairment (Child-Pugh class B and C).

Side Effects

Common side effects include:

  • Fluid Retention / Edema: May cause swelling, especially in the face and midsection.
  • Anemia: Reduced red blood cell count leading to fatigue and weakness.
  • Hepatotoxicity: Elevated liver enzymes; monitor liver function regularly.
  • Decreased Sperm Count: Potential impact on male fertility.

Less Common Side Effects

Although rare, some patients may experience:

  • Nausea
  • Diarrhea
  • Nasopharyngitis (common cold symptoms)
  • Back pain
