Ruxolitinib cream, also known by the brand name Opzelura, contains the active ingredient ruxolitinib phosphate (1.5%). It is a prescription-only, topical Janus kinase (JAK) inhibitor used to modulate the immune system and treat specific skin conditions.

 

Introduction

As a JAK inhibitor, ruxolitinib works by blocking specific signaling pathways (JAK1 and JAK2) in immune cells to reduce inflammation and immune-mediated responses in the skin. This mechanism helps alleviate symptoms like itching and redness in atopic dermatitis and promotes repigmentation in vitiligo.

 

Uses

Ruxolitinib cream is approved for treating non-segmental vitiligo in adults and adolescents aged 12 and older. It is also used for mild to moderate atopic dermatitis (eczema) in non-immunocompromised patients (adults and children 12 years and older) when other topical prescription treatments are insufficient.

 

Common Side Effects

Most side effects occur at the application site.

For Atopic Dermatitis: Common side effects include common cold (nasopharyngitis), diarrhea, bronchitis, ear infection, hives, and folliculitis.

For Vitiligo: Common side effects include acne, itching, or redness at the application site, common cold, headache, urinary tract infection, and fever.

 

Serious Warnings: While topical use has low systemic absorption, the class of oral JAK inhibitors has boxed warnings regarding serious infections, major adverse cardiovascular events, blood clots, cancer (especially skin cancer), and increased mortality risk. Healthcare providers will monitor for these potential risks.

·        Store at room temperature, ideally between 68°F to 77°F (20°C to 25°C).

·        Keep it protected from light and moisture.
